pathophysiology of upper gastrointestinal bleeding

"Journal. DIGESTIVE DISEASE" 1989 03 Collection for the latest gastric bezoars: classification, pathophysiology and treatment of progenitor Yi Abstract: Fecal the stomach is gastric foreign body coagulation. Back in the late 19th century to have surgical removal of gastric bezoar report. Now, as the incidence of gastric bezoars in the stomach after resection of up to 5 ~ 12%, it is of great significance. Clinical symptoms: Most patients (80%) had mild epigastric discomfort, but also a sense of fullness, nausea, vomiting, difficulty swallowing or upper gastrointestinal bleeding, many patients v. fullness morning, one-third of the patients had weight loss. Fecal Category: According to the source, gastric bezoars is divided into four categories: phytobezoars, hair bezoars, Milk quality and medication or food bezoar bezoar mass. Phytobezoars the most common, which was formed in the undigested food material, such as cellulose, hemicellulose, lignin and fruit tannin. Gastrectomy (BI â…  and type â…¡ Bi), the diabetes, myotonic dystrophy, or paralysis of the stomach so the patient's stomach and digestive motor function impairment, often occurs phytobezoars. Severe upper gastrointestinal bleeding (upper gastrointestinal bleeding, UGIB) often complicated by acute myocardial infarction (acute myocardial infarction, AMI), and because patients in severe UGIB cardiac failure to check often for missed AMI. These two diseases At the same time is often manifested in the presence of a unique syndrome, different from the simple UGIB or AMI, but also in the regular examination and treatment, there are many contradictions. (1) The clinical presentation is different from the separate existence of any disease. For example, AMI's Signs and symptoms may be masked by severe UGIB, unless for special examination, or AMI will be missed. (2) pathophysiology is also different. For example, a large number of UGIB due to low blood volume, hemodynamics and myocardial hypoperfusion damage can trigger AMI. (3) there is a special diagnostic problem. Although endoscopy UGIB is safe and generally applicable, but in post-AMI endoscopy to cardiopulmonary complications because of the potential had to carefully consider their safety. Similarly, In cardiac catheterization after AMI because the operation of the anticoagulant therapy would UGIB worse. (4) treatment is very difficult. For example, AMI thrombolytic therapy will increase the anticoagulant UGIB. Similarly, UGIB patients after pituitary vasopressin or endoscopic injection of epinephrine will increase myocardial ischemia. (5) prognosis was significantly different, the mortality rate is clearly higher than mere UGIB or AMI. of: Luo Nanhui Author: Air Force General Hospital emergency department, Beijing, 100036 Title: Air Force General Hospital of English Title: JOURNAL OF GENERAL HOSPITAL OF AIR FORCE, PLA, the volume (of): 2003 19 (1) Key words: R542.22 R573.2 Key words: myocardial infarction peptic ulcer bleeding Machine Standard Classification: R57 R5 machine marked Keywords: upper gastrointestinal bleeding endoscopy gastrointestinal bleedingmyocardial infarction thrombolytic therapy for acute myocardial infarction symptoms and signs of cardiac catheterization, cardiopulmonary complications of endoscopic injection of pituitrin pathophysiology diagnosis of many contradictions Check the clinical manifestations of myocardial ischemia and mechanical damage adrenaline missed anticoagulation Fund Project: DOI: References (15) KONDO Y.
